Eligible Beneficiaries

Eligible beneficiaries (typically age 65 or older) with the following health insurance plans may be covered for the Annual Wellness Visit (AWV) and other procedures: Private Health Insurers, Affordable Care Act, Medicare, Medicare Part B and Medicare Advantage so long as the provider accepts assignment.
